Compassionate Home Health Care: As a Licensed Practical Nurse (LPN) with Compassionate Home Health Care, you'll use your expertise to provide professional nursing care to geriatric patients with the overall goal of helping them to regain independence. We collaborate to provide an interdisciplinary approach to caring for our patients in home or senior living facilities in accordance with the Plan of Care, agency guidelines and the State Nurse Practice Act. Responsibilities: Provides skilled nursing visits for clients as directed by the Case Manager and follows the care plan established by the Registered Nurse. Recognizes and reports changes in client condition to the Case Manager, Physician, and Clinical Supervisor as directed. Communicates information on a timely basis, consistent with Agency policy and urgency of the situation. Communicates verbally, as needed, and through complete and accurate documentation in the clinical record.
