We collaborate to provide an interdisciplinary approach to caring for our patients in home or senior living facilities in accordance with the Plan of Care, agency guidelines and the State Nurse Practice Act. Oh, and we offer competitive pay in a pretty excellent work environment! This is a Saturday/Sunday position performing Start of Care visits for home care patients. You are willing to perform Start of Care openings on both Saturday and Sunday as needed. You are willing to travel within a designated area. You are passionate about working with elderly and helping them achieve great outcomes. You love to be part of an interdisciplinary team, laugh and have fun while working hard to provide ridiculous service. Responsibilities and Duties: Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters; completes comprehensive assessments including OASIS data set within forty-eight (48) hours of referral. Maintains client records; develops and/or follows an individualized Plan of Care. Guides and teaches the patient in prescribed activities directed toward improving function (physical/mental) and independence. Communicates plans and changes to the Clinical Supervisor and attends required meetings. Documents all findings, plans, interventions, and outcomes and submits documentation per Agency policy.
