Responsibilities
RESPONSIBILITIES:
* Provides accurate and timely assessments of the family/patient enabling identification of needs.
* Formulates and implements the individual the plan of care with medical director and interdisciplinary team based on the patient/family assessment and hospice standards of care. Manages discomfort and provides symptom relief.
* Evaluates the effectiveness of the interventions through regular monitoring, observations and reassessment of the patient/family situation. Makes revisions as needed.
* Monitors the quality of care provided for the patient through participation in quarterly monitors according to the Quality Assurance Plan.
* Participates in the orientation of employees ensuring understanding of policy and procedure of hospice program.
* Displays sensitivity in discussing the reality of impending death and hospice care with patients, families, physicians and community groups.
* Assists in the interview process by evaluating clinical skills of nursing staff.
Performs activities as necessary to support hospice function including:
* Continual review of patients charts for appropriate and timely documentation to ensure quality and continuity of care.
* Participating in discipline and monthly staff meetings.
* Obtaining, utilizing, and maintaining equipment and supplies in a cost:effective manner.
* Providing personal care to patients as needed.
* Participating in bereavement follow:up per policy.
* Completes staff assignments in conjunction with the manager.
* Procures the necessary supplies, equipment, and support necessary for complete patient care.
* Assists with scheduling and patient care coverage.
* Completes referrals, admissions, and discharges.
* Documentation in the patient medical record is concise, timely, objective information which impacts patient care.
* Continually updates knowledge of hospice policy and procedures.
* Accepts responsibilities for the quality of care delivered under his/her direction.
* Maintains current knowledge of all state and federal regulations and requirements regarding hospice care.
* Teaches, supervises and counsels the patient and family members regarding the nursing needs and other related problems of the terminally ill.
* Educates patient and family about disease process and end of life care.
* Demonstrates knowledge of team interdependence by adjusting daily schedule to accommodate needs of other patients in the program, assisting other staff members with difficult situations, and promoting team unity by sharing responsibility of patient care for all patients.
Qualifications:
QUALIFICATIONS:
* Graduate of an accredited school of nursing.
* Understanding and acceptance of the Hospice concept of care.
* Must maintain current Michigan licensu
